Complete requests will be processed then mailed to the address provided below. Incomplete forms will delay the processing of your request.

Please note that Sacramental Records are only available to:

• The person named in the record
• A parent or legal guardian of the person named in the record
• (ONLY if the person is under age 18. If the person is unable to request the
  certificate themselves or is otherwise incapacitated, please include an
  explanation in the Additional Notes section of the request form)
